Office Policy
ORTHOPEDIC ASSOCIATES OF HAWAII, LLP strives to provide superior service to our patients. In doing so, we must inform you of the following company policies which are necessary to reduce administration costs and increase efficiency within our practice:
DISABILITY/MEDICAL FORMS
There will be a charge for the completion of disability and other medical forms. The forms will be completed in 7-10 days, and you will be called when the forms are ready for pick up at our Cashier’s Office.
NO SHOW/CANCELLATIONS CHARGE
We kindly ask that you notify us 24 hours in advance of a cancelled and/or rescheduled appointment. Failure to do so may result in a No Show charge.
CREDIT CARD PAYMENT POLICY
Upon completion of an authorization form, ORTHOPEDIC ASSOCIATES OF HAWAII, LLP will automatically charge the credit card on file for services as described in the Automatic Credit Card Payment Authorization Form.
