Pre-Registration Services:
If you have a printer available, OAHI’s online pre-registration process is a time-saving option that allows you to pre-register at your convenience. Simply print out the “OAHI New Patient Pre-Registration Forms Packet” from the Patient Forms (Downloadable) area below and complete them prior to visiting our Registration Department upon your first visit.
If you need assistance, please call us to help you through the process. We are available Monday through Friday, 8:00 am – 4:30 pm. Please call us at (808) 536-2261 and ask for Registration Services.
What you will need to pre-register:
- Picture ID (Driver’s license or State ID)
- Insurance card information
- Appointment date and time
- Type of appointment (knee exam, knee pain, shoulder exam, shoulder pain, etc.)
- Personal Contact information
- Physician’s name
If you are seeing us for a work related injury or auto related injury, we also need:
- Insurance carrier’s name
- Adjuster’s name
- Adjuster’s phone number
- Claim number for your injury
- Date of injury or Date of accident
Patients with HMSA, HMA, HMAA and Summerlin Insurance:
- Please download and complete the appropriate Third Party Liability (TPL) form in the Patient Forms (Downloadable) section below. To avoid delay by your insurance company, these forms should be filled out in advance for any injury type of problem.
Once you have completed your forms, and signed off on them, be sure to place them in a safe/secure location until the time of your appointment, when you will present them to our personnel at our Registration Services counter.
