FAQs

General

Do I need a referral?

Depending on your insurance you may need to get a referral from your Primary Care Physician to see one of our specialists. Please contact your insurance carrier if you are unsure if a referral is needed. The following plans commonly require referrals:
  • HMO plans
  • Medicaid Quest plans
  • Medicare Advantage HMO plans

Where is parking available?

Plenty of convenient parking is available in our building. Enter via Young or Beretania Street. Once you’ve parked, take the elevators to the ground floor of the parking structure and follow the signs guiding you to the entrance of our office building. We are on the 7th floor of the office building, in suite 750. There are no other bridges connecting the parking structure and office building, only the ground floor provides access to both.
Your first hour of parking is free with validation. The rate is $1.50 per half hour after that.

What should I bring to my first appointment?

You must bring an official photo ID (State ID/Driver’s license/Military ID) and your insurance card in order to be treated. Ask your specific doctor if additional documents are necessary. Please be prepared to provide a detailed account of your medications, recent surgeries, and medical history. These facts are critical to helping us to treat you safely and efficiently.
